If a non-combat NPC (Kabuto, Shizune, Tsunade, etc) is hit by Iron poison, then that poison debuff will remain forever. I'm unsure if it continues to deal damage, but the overlay remains there. If it continues to deal damage, then it might cause a drag on the server.
